I’m a noob at COT stuff, but how is this indicator applied to bitcoin? Do exchanges report bitcoin positions of it’s traders? If so, that’s just in the US, right?

iCOT on the other hand is intraday indicator : it is pure technical indicator built by reverse engineer COT data.
