Are there any USA Trader Approve Prop Firm using MT4 & 5
What do you mean? Are you asking if any company is doing MT5 for US traders? if they accept them?
I think there can’t be, because Metaquotes withdrew all the prop firms’ licences to use MT for US customers some months ago.
Since then, some prop firms have announced that they now offer a new platform called “Platform 5”, and it’s claimed by some people to be “similar to MT5”. But it has some truly awful reviews and most people seem to be saying after one attempt that they’ll never touch it again.
Futures prop firms are far better and more reliable than CFD prop firms anyway.
Everyone I know who has got through a CFD prop firm evaluation and into a “funded” account (they’re not really funded, of course!) has used c-trader, which is far, far superior to any MT platform but also, sadly, not available to US residents.
This is all because CFDs are illegal in the US, of course.
It’s much, much better and more sensible, for Americans, to do a futures prop firm try-out and to use Tradovate.
They’re much better and more honest and more reliable companies to deal with (apart from “Apex” who are really horrendous fraudsters but probably won’t be around for much longer anyway, by all accounts).
Are all accounts with them considered prop firm accounts? Or do they have I guess regular accounts, where you just fund with your own money?

Who is “they”/“them”?
Are you confusing “platforms” with “brokers”? Or confusing “spot forex” with “CFDs”?

You mentioned the futures prop firm Tradovate. I’m in the US.
Tradovate is not a prop firm. It’s a trading platform (and a very good one).
Almost all the futures prop firms offer the Tradovate platform among the choices available for their customers to use. (Free).
The forex/CFD prop firms don’t offer Tradovate because it’s a futures trading platform and isn’t used for forex/CFDs.
The Tradovate company (which is owned by another company called NinjaTrader) is also a brokerage in its own right, and they have their own independent customers who trade their own futures accounts too, as well as the futures prop firms’ customers who are using Tradovate because so many futures prop firms also happen to use it.
